Three ways to order our certificates.

1. Postal Mail: Print and complete our order form and mail with payment enclosed to:

2. Fax: Print and complete ourorder form and fax to 1-918-665-3300.
3. Phone: If paying by credit card call 1-918-664-8604 and have the information from our order form ready.
4. Visit Our Studio: we are located on the southwest corner of 41st Street and Memorial, Tulsa, Oklahoma.

Gift Certificates will be mailed to your recipient by the U.S. Post office within two business days of receipt. Confirmation and certificate number can be e-mailed to you. The certificate is redeemable for products and services only at Tulsa Stained Glass, 7976 E 41st Street (41st and Memorial), Tulsa, OK. 74145. The certificate is good for 90 days from the date we received your order .
